Atrius-authored ValueSet for CQL valueset "Hospice Encounter". Four SNOMED CT International admission concepts are kept. HCPCS hospice-service codes are dropped. HL7 RoleCode HOSP is added so HIS-stamped hospice encounters (and the CMS165 excl-hospice fixture) match. VSAC OID 2.16.840.1.113883.3.464.1003.1003 is provenance only.
